For several months, the topic of hospital finances has regularly returned to public debate. First, reports regarding the very high earnings of Dawid Kacprzyk, a contract doctor at the South Hospital in Warsaw, echoed widely. Later, similar reports began to appear regarding other facilities as well. Each such story raised questions not only about the level of remuneration, but above all about the method of spending public money originating from the National Health Fund. The Ministry of Health does not indicate in the justification for the draft that the proposed changes are a response to these events. However, the document shows a clear direction\u2014the state wants to have more detailed data concerning employment costs in hospitals and use them when evaluating the quality of facility management.<\/p>\n
The draft regulation of the Minister of Health (MZ1926) provides for the addition of three new managerial indicators that will relate to personnel costs. Hospitals will report these metrics.<\/p>\n
Thanks to this, the ministry will be able to more easily compare personnel management methods between individual hospitals and observe how the expenditure structure changes throughout the entire healthcare system.<\/p>\n
The draft coincided with one of the largest debates on hospital financing in years. At the center of attention were high contracts for some specialist doctors, which in individual cases exceeded one million PLN annually. Following media publications, inspections conducted by the National Health Fund and other institutions began. The new regulation does not give the minister tools to determine remuneration levels. Instead, it creates a system that allows one to see how large a share of a facility’s budget is taken up by personnel expenditures, what portion of money goes to salaried positions versus contracts, and what the medical cost structure looks like. Healthcare management experts have pointed out for years that without comparable data, it is difficult to evaluate the efficiency of hospital operations. Two facilities can perform a similar number of services while incurring completely different employment costs.<\/p>\n
The shortest answer is no. The draft regulation does not provide for changes to remuneration or hiring rules.<\/p>\n
However, the method of gathering and analyzing data intended to serve the evaluation of hospital management quality is changing. The new indicators will become one of the elements of the quality monitoring system conducted by the Ministry of Health.<\/p>\n
The most interesting element of the draft is that the ministry does not focus solely on the level of remuneration. The structure of employment costs is of much greater significance. If two hospitals receive similar funding from the National Health Fund, but one allocates a decidedly larger part of its budget to personnel, the new indicators will allow this to be noticed. The situation will be similar in the case of facilities basing their activity almost exclusively on civil law contracts or, conversely, employing the majority of staff on regular contracts. The ministry indicates that the data is to support the evaluation of management quality and enable the comparison of medical entities according to uniform criteria.<\/p>\n
The draft is at the legislative work stage. After the completion of consultations and interministerial agreements, the regulation can be signed by the minister of health and enter into force. If the proposed solutions are maintained, all hospitals covered by the quality evaluation system will transmit data according to the new indicators. This will allow for the creation of more detailed summaries concerning employment costs and the comparison of personnel management methods between facilities.<\/p>\n
