To obtain the ID card, an application must be submitted to the poviat or municipal disability evaluation board appropriate for the place of residence. The application for the ID card can be submitted by the person with a disability or their legal representative. The mLegitymacja ON can be used after the disability certificate becomes legally binding. For the document to be made available to a legal representative, their data must be included in the application for determining disability or the degree of disability. If they were not entered there or are not in the disability evaluation system, the legal representative must submit a relevant application to the poviat or municipal disability evaluation board.<\/p>\n
The issuance of the first ID card is free of charge. Its electronic version available in the mObywatel application is also free. A fee must be paid only in the case of issuing a duplicate ID card after its loss or destruction, which currently amounts to 15 PLN. On the other hand, issuing a new ID card due to a change of surname, PESEL number, appearance, or supplementing the document with the degree or symbol of the cause of disability is free of charge.<\/p>\n
The validity period of the ID card depends on the type of certificate and the age of its holder. Individuals who have reached the age of 60 receive an ID card valid for the duration of the certificate. If the certificate has been issued permanently, the ID card will also be indefinite.<\/p>\n
