Rynek Zdrowia reports that the error occurred one month ago when two patients with the same surname were present at the facility. The mistake took place during the embryo identification and thawing stage, resulting in an embryo transfer involving the incorrect couple.<\/p>\n
Embryologists detected the error rapidly, and the hospital confirmed that no pregnancy occurred due to safety procedures. The staff member responsible for the incident has been removed from duty.<\/p>\n
The hospital officially reported the incident to the Ministry of Health as an adverse event. The ministry is now reviewing documentation and conducting an inspection to determine if the error stemmed from negligence or a systemic failure.<\/p>\n
