How to Apply for a Free Sanatorium Stay

The application process is straightforward, beginning with a PR-4 form completed by the applicant’s primary care physician. This form can even be submitted alongside a sick leave certificate.

ZUS Decision-Making Process

The final decision rests with a ZUS medical examiner or physiotherapist, who may conduct a personal examination or base their decision on submitted documentation. Bringing comprehensive medical records – test results, hospital discharge summaries, and treatment history – is recommended.

If approved, applicants will receive a notification with their rehabilitation dates and location. The decision process typically takes 3 to 8 weeks.

Appealing a ZUS Sanatorium Denial

A denied application isn’t final. Applicants have one month from the decision date to appeal to a district court, submitting a written or oral appeal through ZUS.

Rehabilitation Duration and Coverage

Standard stays last 24 days, but the rehabilitation center physician can adjust the duration with ZUS approval. A post-treatment summary detailing the patient’s health status is generated upon completion.

Cost Coverage in ZUS Sanatoriums

Unlike NFZ sanatoriums, ZUS-funded rehabilitation covers treatment, accommodation, and meals. ZUS also reimburses travel costs using the most affordable public transportation option, potentially making the entire stay free.

Rehabilitation covers a wide range of conditions, with stationary rehabilitation commonly prescribed for issues such as musculoskeletal problems and circulatory system disorders.

Ambulatory rehabilitation – where patients commute for treatments – is available for musculoskeletal and circulatory conditions.

Rehabilitation Program Components

The program is individually tailored and may include physiotherapy, hydrotherapy, kinesitherapy, massage, and other therapeutic interventions. It’s important to note this is intensive treatment, not simply rest.
