Doctors to Track Home Health Data Under New 2026 System

Starting by December 2026 at the latest, a new Home Medical Care service will allow seniors and chronically ill patients to log vital health measurements at home for physician review.

New Tool for Seniors and Chronically Ill Patients

Seniors and chronically ill patients will soon gain a new health monitoring tool. The Home Medical Care (DOM) service will allow blood pressure or blood sugar measurements to be recorded at home, and the doctor will see their history during a visit. This means that assessing the patient’s condition will not have to rely solely on a single measurement taken in the office. The service is to be available on the Internet Patient Account and in the mojeIKP application.

How Does Home Medical Care Work?

So far, during a visit, the doctor often relies on results from that specific day or information provided by the patient. Home Medical Care is designed to provide a much broader picture. A patient qualified for the service will perform measurements indicated by the doctor at home, and their results will be saved in the system. As a result, during the next visit, the doctor will be able to analyze not a single result, but the history of measurements and changes occurring over time. This can be especially important for people whose health requires regular monitoring of specific parameters.

Blood Pressure, Sugar, and Smartwatch Data

Using DOM will not involve simply signing up for the service on one’s own initiative. The process will begin during a medical visit. The doctor will qualify the patient for the service and then determine what measurements should be performed, how long they should be performed, how often measurements should be taken, and at what times they should be carried out. The patient will then perform the measurements at home and save their results on the Internet Patient Account (IKP) or in the mojeIKP application. The data will later be available to medical personnel using the system.

Data from Various Medical Devices

DOM will not be limited solely to results manually entered by the patient. Data may come from medical devices such as blood pressure monitors, glucometers, and other medical measurement devices. It will also be possible to use data from devices cooperating with Google Health or Apple Health, including certain smartwatches, scales, or activity and parameter-tracking bands. The patient will also be able to enter the measurement result manually. The range of available functions will depend on the device and how it is integrated with the system.

Target Audience and Qualification Rules

The service is primarily targeted at patients who require regular health observation. Among the groups indicated by the e-Health Centre are listed. However, this does not mean that every patient from one of these groups will automatically receive access to the service; the doctor will qualify the patient and determine the scope of measurements during the visit.

Physician Access to Historical Patient Data

The most important change is not the electronic recording of results itself, but the fact that the doctor will be able to see a sequence of measurements taken between visits, allowing a broader look at the patient’s condition beyond a single office exam. DOM is intended to support the doctor in decision-making, but it does not mean automatic, round-the-clock patient monitoring. Therefore, DOM should not be treated as a replacement for a doctor’s visit or an emergency system.

Platform Integration via IKP and mojeIKP

Patients will use the service through tools they already know, as DOM is to be available on the Internet Patient Account and the mojeIKP app. Medical facilities will need to integrate with the service, allowing personnel to use DOM via the free Gabinet.gov.pl application or other integrated office systems.

Launch Timeline and Legal Framework

There is currently no single specific date when all patients will be able to use DOM. According to Article 10 of the act, the Home Medical Care system is to be launched no later than December 1, 2026, as the act entered into force on July 1, 2026, giving a maximum of 5 months to launch the system.
